Kids on the waiting list may NOT get to play. Click Here for Age Division Birthdays To sign up, Players need to complete the 4 steps below: 1.) Print and fill out the following form: Youth Registration Form (PDF File) 2.) Include a Check or Money Order payable to Cleveland Soccer Club in the amount of $35 3.) All NEW players must include a state certified birth certificate. If you played last season, your birth certificate should still be on file. 4.) Take form, check, and birth certificate to: THE BODY SHOP @ 110 S Broadway (Hours Mon-Fri 9:00am to 5:30pm Sat 9:00am-Noon) or THE BROADWAY TWIN @ 300 S Broadway (Movie Times Tuesday through Saturday 7:30pm and Saturday and Sunday at 2:00pm) or you can mail to: (Must be RECEIVED by Jan 27, 2006) Cleveland Soccer Club REGISTRATION QUESTIONS: Call Heather Boyles at 243-7708 or Lori Forbes @ 358-1142 We need Coaches and Referees!
